Scientific Games received the largest fine, a $100k penalty.  It allegedly ran three unapproved versions of three games from January to March of this year prior to conducting an audit.  That audit revealed an issue with the games in question.

The DGE imposed a $5k penalty on PokerStars as the company lost poker hand history data for two hours.
